Commentary

I have had essential evidences to show that I was able to practise my profession as a social worker in an organisation which is the medical setting. Being promoted one step higher position in the hospital where I worked had been a great opportunity for me. I was able to supervise minimum number of staff in the medical social service and contribute for the good and welfare of the service as whole. Apparently, I was able to use my capacity to help solve patient’s problem, which directly and indirectly affects their medical condition while in the hospital. I was able to help and support admitted patients to adjust to the hospital situation, its scope of service, limitations, policies, rules and regulations. I was able to coordinate, do networking and linkages with the different existing government and non-government organisations whenever need arises for the good of the service and patient’s welfare as well.

How this shows my professional values

Being an accountable and effective member of the organization in the medical setting, I could say that I was able to contribute and demonstrate my social work principles, knowledge, skills and values. I was able to perform my roles and functions as an efficient and effective medical social worker in accordance with the mission and vision of the hospital organization, which is mainly to achieve the highest attainable patient care.

Core Competence 6: Develop Professional Competence

6. Ability to make decisions

At this point in time, I can say that I was able to develop professional competence because I applied methods of social work in the hospital as well as the skills, knowledge and values in social work practise.

I have had a long period of time practising my social work profession in the medical setting. I was promoted in the service and used my skills and knowledge in supervision. I was able to assist and supervise my subordinates and discuss some cases of individual patients and monitor them in social action and research activities.

Later, I decided to enrol in the post graduate studies but unfortunately I have got only 18 units in social work at the same school where I finished my undergraduate studies. I decided to apply for an optional retirement in the year 2003 because I was determined to work here in the UK as a social worker. During that time, I was aware that it was going to be more challenging and different, and now it is happening. However, I am decided to continue meeting challenges and try the best that I can by using the social work principles, values and knowledge I have got as a professional social worker. By this means, I could develop myself more fully as a competent social worker here in the UK.

How this shows my professional values

Having decided to undergo and perform continuous education shows my professional values and principles as a social worker. I believed my decision to practise my profession here in the UK will surely be a challenging opportunity for me. By applying the social work principles, knowledge, skills and values I have got through education, training and experience in my country, I am very sure I would be able to work here in UK more competently.

Core Competence 1. Communicate and Engage

Practice requirement(s)

1.Ability to network and form effective working relationships with and between individuals, agencies, community resources, volunteers and other professionals.

Commentary

I have had an evidence to show that I met the above practice requirement while working as member of the team in the hospital. I was able to form effective working relationships with my colleagues and other professional services. Networking and linkages with other government and non-government agencies were practiced in the hospital where I worked. The Medical Social Services served as channel of blessings to all patients who were abandoned, neglected, those who were victims of rape, those who were homeless and lastly those patients who could not afford to pay the total amount of hospitalization incurred.

By making social diagnosis of patient admitted in the medical centre, careful planning and sufficient support from different allied professions, with successful discharge and transfer to appropriate placement of patient has been attained. Example: Patient Mr. X was admitted to the hospital on 14 Nov. 2002 due to CVA Infarct. He was an orphan and has no home to return to in the city. Social Case Study abstract, referral letter and coordination through telephone calls were done with a certain government agency for temporary shelter and continues management of patient’s case.

How this shows my professional values

Through networking and referrals with other existing resources in the community, I was able to assist patients admitted in the hospital, by respecting and valuing thier uniqueness and diversity, improve and build thier own strengths. I was able to identify, analyze and take action to the existing problem of Mr. X and promote his right to choice that led to improve his quality of life.

Commentary

I have an evicence to show that I met the above practice requirement. An example is a case of a patient I handled who was admitted in the hospital due to vehicular accident, that caused him to have Femoral Fracture in the neck. Upon my assessment and evaluation, it was found out that his relatives did not know his whereabouts. He was provided with all the services needed for his operation through my collaboration with the doctors, nurses and other professional services in the hospital where I worked. I was able to locate his relative specifically his wife by contacting a government agency, to inform that the patient is in need of huge amount of money for operation and other hospitalization expenses. The patient’s wife was encouraged to collaborate and participate in the problem solving process. I was able to refer and bring patient’s case to a certain government agency for financial support.

How this shows my professional values

I was able to use my social work capacity by supporting my patient and his wife identify thier strenghts, promote thier rights and meet thier responsibilities. I managed to assist them in changing and improving thier quality of life.

Commentary

The following evidence showed that I have met the above practice requirement. I handled a case of a female patient who was 50 yrs old, married and a plain housekeeper. She was admitted to the hospital on 12 July 2003 due to nasopharyngeal carcinoma, wherein she needed different medical procedures and physical examinations before the expected operation. My assessment and evaluation proved that patient’s family income was not even enough to defray cost of thier daily living because of poverty. I advised and supported patients’ daughter to look for other resources  to be able to help patient pay the needed procedures to be done. The daughter who was jobless was referred to a private agency to hold a temporary job and fortunately she was accepted to work in a certain restaurant. Patient’s case was brought to the attention of the doctors, nurses and other ancillary services of the said hospital to carry out plans for patient’s medical treatment. I constantly collaborated with the patient and his family as well as the attending physician who was holding the case and other professional health services to be able to obtain successful care and support for his operation.

How this shows my professional values

By achieving this practice requirement, I was able to analyze and illustrate the effect of patient’s illness to social, physical, emotional and financial implications to the patient himself and his family. I was able to work in partnership with the professional health services available in the hospital as well as working with patient and his family. Thereby successful packages of care, support, protection and control were met and developed.

Core Competence 4: Intervene and Provide Services

Commentary

By working directly with a certain patient I handled in the hospital, I managed to intervene and provide services which showed the above stated practice requierment. I have an example of a patient who was admitted in the hospital because of chronic ailment. Multiple problems were identified after having been treated by the assigned physician through an intensive case study. Patient needs financial assistance for his medical examinations and treatment since his relatives did not know his whereabouts and could not afford his hospital expenses as well. He has no relatives looking after his personal needs and comfort in the hospital since he was alone who came to the city, purposely to work and be able to support his family in the province. The case was brought to the attention of the hospital medical administration and ancillary services to provide him temporary financial support while waiting for his relatives to come over. I also referred and coordinated the case to the two government agencies, to be able to locate his relatives and help him financially.

How this shows my professional values

Through intervention and working with patient I was able to achieved provision of care, support and protection for him, with his full cooperation and participation, I was able to analyze and take action immidiately to the identified problems since his wife came before his discharged from the hospital. Coordination with the different existing resources in and out of hospital was done. In doing so, I was able to promote patient’s rights as a good member of the society. I was able to identify, recognize and develop patient’s potential and capabilities as well as his own family to become useful member of the community where they belong.
